MAPP Tool Repository

The MAPP Tool Repository features tools by NACCHO, MAPP communities, and national and regional partners including tools for planning your MAPP process, engaging partners, elevating health equity action, implementing the MAPP assessments, gathering data, developing strategic priority areas, creating shared action plans, communicating with stakeholders, and other critical components of the community health improvement process. The tools are vetted by the NACCHO MAPP Team and meet high quality standards that include furthering health equity, embracing collective action, and encouraging rigorous quality improvement and evaluation.
