The Council of State and Territorial Epidemiologists (CSTE) is conducting an assessment to gather information about chronic disease data modernization successes, priorities, and opportunities for advancement. This information will be used to inform a national strategic plan for chronic disease surveillance modernization. While each local health department has unique challenges and opportunities within chronic disease surveillance, they would like to more broadly understand and convey the diverse needs of local jurisdictions so that their strategic plan is representative of all levels of public health.
The deadline to complete the assessment is 11:59 pm EDT on Friday, April 19, 2024. The estimated time to complete the assessment is 15-20 minutes.
