CVH CoP Webinar: Leveraging Partnerships to Promote Medication Therapy Management in Los Angeles

NACCHO in partnership with the CDC’s Division for Heart Disease and Stroke Prevention hosts the Cardiovascular Health Community of Practice (CoP). At a national level, this Cardiovascular Health CoP brings together local health departments and partners to spur the dissemination of promising practices and to advance policy, systems, and environmental approaches that have been effective in reducing the burden of chronic disease, specifically by addressing cardiovascular health.

In this webinar presentation, the Los Angeles County Department of Public Health’s Dr. Tony Kuo will provide an overview of local efforts to promote medication therapy management in the clinic and community setting. Special emphasis will be placed on work supported by the Centers for Disease Control and Prevention’s 1817 initiative. Dr. Kuo will discuss the Department’s strategic approach to promoting cardiovascular health which leverages partnerships with experts in the field to effectively stretch resources and achieve greater impact in the community.

Dr. Kuo will also be joined by Dr. Jennifer Kwon who will highlight efforts of the USC School of Pharmacy’s California Right Meds Collaborative, a consortium of health plans, pharmacies, and academic and professional organizations working to improve continuity of care through comprehensive medication management.
