NACCHO in partnership with the CDC’s Division for Heart Disease and Stroke Prevention hosts the Cardiovascular Health Community of Practice (CoP). Representatives from the San Francisco Department of Public Health will present an overview of local efforts to leverage public health, clinical, and community-based assets to support cardiovascular health. Special emphasis will be placed on initiatives launched by the Hypertension Equity Workgroup and the diverse partnerships to advance equity for Black/African American cardiovascular health in San Francisco.At the conclusion of this webinar, participants will be able to:1. Describe how a public health-clinical partnership applied the National Academies of Sciences, Engineering, and Medicine’s 5A framework for tackling social determinants of health to advance food security and cardiovascular health equity2.
