    The Healthcare and Public Health Sector Coordinating Council - Cyber Working Group is collecting information on public health processes to map identify dependency systems in critical Public Health Functions.
     
    Attached, and imaged below is a draft of the Public Health Reportable Disease Dependency Map. Please provide feedback on this map to include missing systems, agencies, or other elements. Narrative explaination:
     
    Some diseases are required by law to be reported to public health.
    The patient generates a medical contact, finding a reportable disease the healthcare provider either: reports the disease to public health via direct contact (email/phone), through disease recording system, or through automatic import from the electronic medical record system to the disease recording system. Local/regional Public Health then contacts the patient for case investigation. Recording this info into disease record system, in some diseases there may be 2 systems used. Disease record system may be owned/operated by state public health. This info is passed to CDC for national records or federal response in rare cases.
