The program combines patient education, home blood pressure monitoring, and ongoing support from Quality Navigators.

Key Strategies
✅ Provided home blood pressure monitors to support self-monitoring
✅ Paired patients with Quality Navigators who offered individualized guidance and follow-up
✅ Delivered education on hypertension, nutrition, physical activity, and proper blood pressure measurement
✅ Created direct communication channels between patients and providers
✅ Screened patients for additional needs and connected them to available resources
Results & Impact
📈 109 blood pressure monitors distributed
📈 45 participants completed the program and received education on blood pressure management, nutrition, and physical activity
📈 Participants lowered their blood pressure by an average of 16.4 mmHg systolic and 10.6 mmHg diastolic
📈 300 patients received screening and support to identify needs that could affect their health
📈 311 patients continue to benefit from receiving blood pressure monitors through the ongoing program
