Continuity of Care Resources Library

This library includes new resources identified after the Blueprint publication.  The resources are organized by folders that align with each strategy of the 7 Blueprint Recommendations.
To access resources in the library:

• Navigate to the library on the lefthand side of the page below and click on the Recommendation folder to view resources saved for each recommendation.

• Click on the (+) button next to each Recommendation to access the various subfolders for specific topic areas (Note: Most resources are organized into the subfolders and not in the main folder for that Recommendation.)

• To search for specific types of resources, use the Search field in the top righthand corner.

  
  

If you do not find what you're looking for, please check the Blueprint document.  Each recommendation has a comprehensive section of useful tools, resources, and stories to assist in implementation.
